Infographic Request Form

Please use this form to request an infographic for your committee.
When you complete the “Description” textbox, please try to provide the following details:
A. Suggested title or header for the infographic.
B. Four to six bullet sentences that will clearly and concisely convey your message to readers.
C. Describe graphics, if any, that might spice up the infographic. Examples include an icon, a photo, chart, table, etc.
D. Hyperlinks – Be sure to include links to websites, etc. if you want them on the infographic.
E. If relevant, references or sources of any information provided by the infograph.
